About The Position

CLFC Healthcare and Communications is hiring a per-diem Certified Nursing Assistant for Wayne County’s on-site, 192-bed skilled nursing and rehabilitation facility in Lyons, New York. The facility is rated High Performing in Long-Term Care by U.S. News Best Nursing Homes 2025 and has licensed nurses on every shift. This W-2 hourly role lets CNAs choose their availability and pick up shifts through the BRIDGE App, with no minimum hours. Shifts are available any day of the week, including 6:30 AM–3:00 PM, 2:30 PM–11:00 PM, and 10:30 PM–7:00 AM; twelve-hour shifts may also be available. Each standard shift includes eight paid hours and a half-hour unpaid meal break. Pay is $25–$28 per hour, with night and weekend shifts paid at the top of the range, weekly pay, and time-and-a-half for overtime over 40 hours and all ten County holidays. A confirmed shift cancelled by the facility two hours or less before its start is paid for four hours. Paid orientation is provided. CLFC arranges and pays for fingerprinting, immunizations, screening, and license verification. The role includes paid sick leave as provided by New York law and professional liability insurance coverage on every shift. CPR renewal reimbursement, support for continuing education and certifications, tuition assistance of up to $1,500 annually toward an LPN or RN program, and a $500 referral bonus are offered through CLFC University, subject to program terms provided in writing before acceptance. A Program Manager is available to assist with scheduling and pay questions. Requirements

Current listing in good standing on the New York State Nurse Aide Registry.
Current BLS (CPR).
Six months of recent CNA experience.
Ability to complete the New York State Department of Health Criminal History Record Check and a drug screen.
Current on immunizations required by New York for nursing home personnel.

Responsibilities

Provide personal care, including bathing, dressing, toileting, and assistance with meals.
Help residents move safely using proper body mechanics and lift equipment.
Take and record vital signs, intake and output, and weights.
Notice and report changes in residents’ conditions to the nurse.
Maintain a clean and safe resident environment and follow infection control practices.
