About The Position

CLFC Healthcare and Communications is hiring a per diem Certified Nursing Assistant for Wayne County's 192-bed skilled nursing and rehabilitation facility in Lyons, New York. The facility is rated High Performing in Long-Term Care by U.S. News Best Nursing Homes 2025 and has licensed nurses on every shift. CNAs provide residents with personal care, assistance with meals and mobility, vital-sign monitoring, and observations that help nurses deliver care. The position offers employee-selected shifts with no minimum hours; shifts may be picked up around other commitments or built into a fuller week. Available shifts include 6:30 AM–3:00 PM, 2:30 PM–11:00 PM, and 10:30 PM–7:00 AM, with twelve-hour shifts when available. Each standard shift includes eight paid hours and a half-hour unpaid meal break.

Compensation is $25.00–$28.00 per hour, paid weekly. Experience determines the rate within the range, and night and weekend shifts are paid at the top of the range. Overtime over 40 hours and all ten County holidays are paid at time and a half. A confirmed shift canceled by the facility two hours or less before its start is paid for four hours. Orientation is paid at the employee's rate. CLFC arranges and pays for fingerprinting, immunizations, screening, and license verification. Paid sick leave accrues under New York law, and professional liability insurance applies on every shift.

Additional support includes reimbursement for CPR renewal, support for continuing education and certifications, and up to $1,500 per year in tuition assistance toward an LPN or RN program. A $500 referral bonus is offered for bringing a nurse or aide onto the roster. CLFC University provides training and development, with program terms provided in writing before acceptance. Shifts are selected through the BRIDGE App, which also tracks licenses and certifications and sends expiration reminders. A Program Manager is available to help with scheduling and pay questions. Requirements

• Current listing in good standing on the New York State Nurse Aide Registry.
• Current BLS (CPR).
• Six months of recent CNA experience.
• Ability to complete the New York State Department of Health Criminal History Record Check and a drug screen.
• Current on the immunizations required by New York for nursing home personnel.

Responsibilities

• Provide personal care, including bathing, dressing, toileting, and assistance with meals.
• Help residents move safely using proper body mechanics and lift equipment.

• Keep residents' environments clean and safe and follow infection control practices.
