About The Position

CLFC Healthcare and Communications is hiring a per diem Certified Nursing Assistant to work on-site at Wayne County’s 192-bed skilled nursing and rehabilitation facility in Lyons, New York. The facility is rated High Performing in Long-Term Care by U.S. News Best Nursing Homes 2025 and has licensed nurses on every shift. CNAs provide residents’ personal care, assist with meals and mobility, take vital signs, and report observations to nurses. This is a W-2 hourly per diem role with employee-selected availability and no minimum hours. Shifts are available any day of the week: 6:30 AM–3:00 PM, 2:30 PM–11:00 PM, and 10:30 PM–7:00 AM. Shifts include eight paid hours and a half-hour unpaid meal break; 12-hour shifts may also be available. Open shifts are posted in the BRIDGE App, where employees select shifts and receive confirmation. Compensation is $25.00–$28.00 per hour, with experience determining the rate within the range and night and weekend shifts paid at the top of the range. Overtime over 40 hours and all ten County holidays are paid at time and a half. Pay is weekly, and orientation is paid at the employee’s rate. If the facility cancels a confirmed shift two hours or less before its start, the employee is paid for four hours. CLFC arranges and pays for fingerprinting, immunizations, screening, and license verification. Paid sick leave accrues under New York law, and CLFC provides professional liability insurance for every shift. The role offers reimbursement for CPR renewal, support for continuing education and certifications, and up to $1,500 per year in tuition assistance toward an LPN or RN program. Employees may receive a $500 referral bonus for bringing a nurse or aide onto the roster. Program terms are provided in writing before acceptance. A Program Manager is available to help with scheduling and pay questions. CLFC also considers rostered staff first when new assignments become available.
