Certified Home Health Aide (CHHA/CNA) - PRN
About The Position
Holisticare Hospice is seeking a Per Diem Hospice Aide (CNA/HHA) to provide hands-on care that brings comfort and dignity to patients at the end of life. Working closely with the nursing team, the aide helps meet patients’ physical, emotional, and personal needs with respect, empathy, and professionalism. The schedule is per diem on weekends, covering Ocean County. Holisticare Hospice describes itself as a mission-driven, patient-centered provider of end-of-life care, supporting patients and their loved ones with comfort and compassion. The role includes following individualized plans of care, assisting with personal care, supporting families, delivering strong customer service, and maintaining accurate documentation in patients’ homes and in accordance with agency and regulatory standards.
Requirements
- High school diploma or GED.
- Valid Certified Home Health Aide (HHA) license or valid Certified Nursing Assistant (CNA) license.
- Current CPR certification if mandated by the state.
- Current driver’s license and vehicle insurance, and access to dependable transportation.
Nice To Haves
- At least six months of experience in hospice or a skilled nursing facility; hospice experience is a plus.
Responsibilities
- Follow the plan of care to support patients’ comfort and well-being, and respond promptly to requests from patients and their families or caregivers.
- Assist patients with personal care, including bathing, oral hygiene, changing bedding and clothing, and shampooing.
- Provide exceptional customer service and work to ensure patient satisfaction.
- Share knowledge with and guide and support families caring for terminally ill loved ones.
- Maintain accurate documentation in patients’ homes and in accordance with agency and regulatory standards.
