About The Position

Sonder Healthcare is hiring a Certified Home Health Aide to provide home health and hospice support in patients’ homes across Pollock Pines, Camino, Placerville, Diamond Springs, Rescue, Cameron Park, and El Dorado Hills, California. This is an independent, field-based, per-diem role with flexible scheduling. The position requires an active HHA certification and at least one year of home health aide experience. The work location is on the road. The aide provides personal care and practical assistance, supports patient safety and mobility, records vital signs and care documentation, and communicates with patients, families, and the healthcare team. Requirements

Active Certified Home Health Aide (HHA) certification.
Valid driver’s license, reliable transportation, and active auto insurance.
At least one year of Home Health Aide experience.
Basic medical knowledge.
Ability to work independently and follow instructions.
Strong communication skills.
Compassionate, patient-focused approach and ability to maintain confidentiality.
Ability to commute to the listed service area in California.

Responsibilities

Provide personal care, including bathing, grooming, and dressing, in patients’ homes.
Assist patients with mobility and safe transfers.
Administer medications as directed.
Monitor and record vital signs.
Prepare meals and assist with feeding.
Perform light housekeeping and laundry.
Maintain accurate documentation.
Communicate with patients, families, and the healthcare team.
