About The Position
The Case Manager works at an advanced level of nursing and applies knowledge of disease management, critical pathways, and the continuum of care from admission through discharge. The role assesses patient needs and facilitates inpatient planning using criteria sets, collaborating with physicians and other healthcare providers to develop safe and appropriate plans of care. Depending on the predetermined assignment, the Case Manager may work in the Emergency Center, Birthing Center, or on medical/surgical floors with inpatient or observation patients.
Requirements
- Bachelor of Science in Nursing (BSN).
- Michigan RN license.
- 3–5 years of current experience in case management, utilization review, or a related field.
- Excellent customer service experience.
Responsibilities
