Registered Nurse-RN CASE MANAGER - St. Mary Home Care
About The Position
St. Mary Home Care has a full-time opening for a Primary Care Nurse providing home care services in the Levittown, Bristol, and lower Bucks County area. The Registered Nurse provides primary nursing care to home-based patients as ordered by the physician and in accordance with organizational policies and procedures. The RN completes initial assessments and develops care plans in collaboration with the Population Health Case Manager, provides functional support and supervision to Home Healthcare Aides and LPNs as needed, and delivers therapeutic interventions and overall care management to support excellent client outcomes and patient satisfaction. The role offers one-to-one, patient-centered care in patients’ homes, flexible scheduling opportunities, supportive colleagues, professional development, and access to advanced remote-monitoring technology through the Home Care Connect virtual care program. The position requires participation in a weekend rotation once monthly and work on two holidays annually.
Requirements
- Graduate of an approved nursing education program.
- Current Registered Nurse licensure in the state of Pennsylvania.
- Current driver’s license and reliable transportation.
- Ability to work a weekend rotation once monthly and two holidays annually.
Nice To Haves
- One year of experience as a professional care nurse, with home care experience preferred.
