About The Position
St. Mary Mercy Hospital is seeking a Case Manager to work in Case Management/Emergency Department. The role requires advanced nursing practice and an understanding of disease management, critical pathways, and the continuum of care from admission through discharge. The Case Manager assesses patient needs, facilitates inpatient planning using criteria sets, and collaborates with physicians and other healthcare providers to develop safe and appropriate plans of care. Depending on the predetermined assignment, the Case Manager may work in the Emergency Center, Birthing Center, or on medical/surgical floors with inpatient or observation patients.
Requirements
- Bachelor of Science in Nursing (BSN).
- Current Michigan RN license.
- 3–5 years of current experience in case management, utilization review, or a related field.
- Excellent customer service experience.
