About The Position

Under the direction of the Program Manager, the Case Manager will provide compassionate, client-centered case management services, engaging individuals experiencing homelessness, connecting them to resources, and supporting them in achieving meaningful outcomes and greater stability. This role requires a commitment to the Mission, Vision, and Values of the brothers of St. John of God as witnessed through Camillus House in serving the needs of the underserved populations. The mission is rooted in the compassionate hospitality of St. John of God, aiming to improve the quality of life for those who are vulnerable and homeless in South Florida through a continuum of housing and supportive services. The values include Hospitality, Respect, Quality, Spirituality, and Responsibility. The vision is to support the values of the Hospitaller Brothers of St. John of God through efforts to enhance positive outcomes that result in sustainable housing, adapting to meet new challenges facing the homeless in contemporary society.

Requirements

  • Ability to understand and practice the mission of Camillus House.
  • Bachelor’s Degree preferred in Social Work, or related field; any appropriate combination of education, experience and/or certification will be considered.
  • A minimum of 2 years of experience in the same or similar job role is preferred.
  • Proficiency in oral/written English; Spanish preferred; additional languages a plus (i.e., Creole).
  • Proficiency in computer (Word, Excel, other data collecting programs).
  • Successful completion of Toxicology test, L2 fingerprinting, local background screening, and OIG reference check.
  • Valid FL Driver License with a clean driving record.

Nice To Haves

  • Spanish preferred; additional languages a plus (i.e., Creole).
  • For Case Managers in Behavioral Health programs (mental health and substance abuse), prior experience working with those populations, or specific training/certifications in those areas, is preferred.

Responsibilities

  • Develop individualized housing stability plans in collaboration with client addressing short-term and long-term goals.
  • Be comfortable working directly with highly vulnerable populations to complete thorough needs assessments, work with their assigned clients to develop service plans and goals to address their unique needs and coordinate with inside and outside partners to secure services for the client (e.g. benefits, transportation, training, training, etc.), with the goal of addressing issues and barriers so that they may secure and/or maintain housing stability.
  • Be proficient in oral/written English and have the ability to document client progress and activity.
  • Has prior experience providing case management to vulnerable populations and the homeless in particular.
  • Is familiar with the homeless system of care (e.g. coordinated entry, housing first, HMIS).
  • Is familiar with other services available to assist homeless persons, (Social Security, DCF, Medicaid, etc.).
  • Is familiar with evidence-based models in a social work setting.
