About The Position

The Case Manager is part of UChicago Medicine’s Care Coordination team at Comer Children’s Hospital, located on the main campus in Hyde Park. As a member of the leadership team responsible for developing and maintaining patient care standards, the Case Manager coordinates and oversees patient care primarily during the inpatient phase of treatment. The role partners with the clinical and multidisciplinary teams to provide high-quality care, ensure continuity of care, and support appropriate use of resources.

Requirements

Bachelor’s degree in nursing from an NLN-accredited school of nursing.
Current Illinois Registered Nurse license.
Three or more years of nursing experience.
Associate Degree in Nursing is listed as required education; associate-degree nurses must enroll in a BSN program within six to nine months of hire, make progress toward the degree, and complete it within 30 months.

Responsibilities

Plan, monitor, and coordinate patient care using the plan of care or clinical pathway.
Identify patient and family needs for consultation with multidisciplinary specialists and ensure those needs are met.
Plan and conduct or participate in patient care conferences.
Assess patient discharge needs and ensure completion of discharge-related functions in a timely manner.
Consult with staff nurses to evaluate current patient status and plans of care.
Ensure comprehensive information needed to document the patient's hospital course is available to care team members, and document in the patient record as appropriate.
Perform other duties as assigned.
