About The Position

AMG Physical Rehabilitation Hospital – Lafayette is seeking a full-time Case Manager (LPN). The position makes special services readily available to patients and their families or significant others to support adjustment and continuity of care after discharge. The Case Manager coordinates admission and discharge planning, case communications, interdisciplinary team conferences, insurance relations and managed-care processes, and verbal and written reporting. The role also ensures that regulations regarding patient rights are fulfilled. An LPN is required.

Requirements

LPN license required.
Ability to read and communicate effectively in English.
Excellent time-management skills; organized and able to prioritize.
Proficient computer skills.
Excellent written and spoken communication skills.
Excellent interpersonal skills with colleagues and others.
Comfortable working in a fast-paced environment with multiple cases.
Able to organize and manage large amounts of files, schedules, dates, and information.
Self-directed and able to work without supervision.

Nice To Haves

Six months to one year of discharge-planning experience preferred.
Additional languages are desirable.
