About The Position

Lifepoint Rehabilitation is hiring a Case Manager at Saint Francis Hospital. The role supports patient care through assessment, identification of patient needs, appropriate interventions, care planning, medical-necessity documentation, and quality-improvement assessments. The schedule is 9:00 a.m. to 2:00 p.m., with a minimum of 24 hours and a maximum of 36 hours.

Requirements

• Effective oral and written communication skills in English.
• At least 2 years of social work or case management experience in an inpatient setting is highly preferred.

Nice To Haves

• Current Registered Nurse or healthcare professional licensure as a Physical Therapist, Speech Language Pathologist, or Occupational Therapist is preferred.
• Case Management or Rehabilitation Nursing certification is preferred, such as CCM, ARN certification, ACM, or ANCC Board Certification in Case Management (RN-BC).
• Acute or rehabilitation hospital experience is preferred.
• Additional language skills are preferred.

Responsibilities

• Accurately perform patient assessments and identify patient needs.
• Identify and initiate appropriate nursing interventions.
• Provide care appropriate to the patient's condition and age.
• Complete timely and appropriate medical-necessity documentation in the medical record.
• Complete and revise the Interdisciplinary Care Plan for each patient.
• Perform timely and accurate quality-improvement assessments.
