About The Position

Lifepoint Rehabilitation, part of Lifepoint Health, is seeking a Case Manager for Saint Francis Hospital. The role supports the organization’s mission of making communities healthier by helping provide exceptional patient care. The Case Manager performs patient assessments, identifies patient needs, initiates appropriate nursing interventions, and provides care appropriate to each patient’s condition and age. The position also includes timely medical-necessity documentation, maintaining each patient’s Interdisciplinary Care Plan, and completing quality improvement assessments.

Requirements

• Effective oral and written communication skills in English.
• A current Registered Nurse or healthcare professional license as a Physical Therapist, Speech Language Pathologist, or Occupational Therapist is preferred.
• At least 2 years of social work or case management experience in an inpatient setting is highly preferred.

Nice To Haves

• Current licensure as a Registered Nurse, Physical Therapist, Speech Language Pathologist, or Occupational Therapist.
• Certification in Case Management or Rehabilitation Nursing, such as CCM, ARN certification, ACM, or ANCC Board Certification in Case Management (RN-BC).
• Acute or rehabilitation hospital experience.
• Additional language skills.

Responsibilities

• Accurately perform patient assessments and identify patient needs.
• Identify and initiate appropriate nursing interventions.
• Provide care appropriate to the patient’s condition and age.
• Complete timely and appropriate documentation relating to medical necessity in the medical record.
• Complete and revise the Interdisciplinary Care Plan for each patient.
• Perform timely and accurate quality improvement (QI) assessments.
