About The Position

Delta Landing is a 172-room emergency shelter program within Contra Costa Health Services Health, Housing and Homeless Services Division, in partnership with Heluna Health. Our mission is to provide safe, interim housing with comprehensive services that support adults experiencing homelessness in securing permanent housing and ending their homelessness. We uphold the dignity of every individual and believe housing is a fundamental right, advanced through advocacy, collaboration, and a Housing First approach.

Requirements

  • Strong knowledge of homelessness, housing systems, and barriers to housing stability.
  • Understanding of behavioral health, including mental health, substance use, HIV, and trauma-related conditions.
  • Demonstrated proficiency in Motivational Interviewing, Trauma-Informed Care, and Harm Reduction models.
  • Excellent communication, organizational, and documentation skills.
  • Ability to exercise sound judgment and maintain professionalism in high-stress, fast-paced environments.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Strong knowledge of community resources and service systems for vulnerable populations.
  • Proficiency in computer applications, including HMIS, Microsoft Office Suite, and data entry systems.
  • Ability to engage diverse populations with cultural humility and a non-judgmental approach.
  • Must successfully pass a criminal background and fingerprint clearance in accordance with State of California requirements.
  • Associate’s degree (AA/AS) in Social Work, Public Health, Health Services Administration, or related field required.
  • Minimum of one (1) year of full-time case management experience in housing, homelessness, or behavioral health services; OR Three (3) years of full-time experience working with individuals experiencing homelessness in a social services setting (in lieu of degree), with a High School Diploma/GED.
  • Valid California Driver’s License (preferred for field-based work and transportation of clients) clean driving record, and must be 21 years of age or older
  • Successful completion of criminal background and fingerprint clearance.

Nice To Haves

  • Bachelor’s Degree (BA/BS) in a related field preferred.
  • Bilingual and bicultural (Spanish preferred).
  • Lived experience of homelessness and/or behavioral health system involvement.
  • Experience in street outreach, shelter operations, or clinical case management.
  • Familiarity with Contra Costa County Coordinated Entry and provider network.
  • Experience with HMIS documentation, reporting, and compliance standards.

Responsibilities

  • Establish and manage a caseload of clients experiencing homelessness in a high-capacity (172-room) emergency shelter setting.
  • Conduct comprehensive needs assessments, including psychosocial, behavioral health, medical, housing, income, and legal domains; complete CE Housing Needs Assessment or other coordinated entry tools as required.
  • Develop, implement, and monitor individualized case plans in collaboration with clients; meet with clients on a weekly basis or as clinically indicated.
  • Provide intensive, client-centered case management using Motivational Interviewing, Trauma-Informed Care, and Harm Reduction approaches.
  • Assist clients in obtaining identification and accessing mainstream benefits, including Medi-Cal, SSI/SSDI, General Assistance, CalFresh, and other public resources.
  • Coordinate referrals and services, including but not limited to: Housing navigation and placement, Behavioral health services, Healthcare for the Homeless services, Substance use treatment (AOD), Legal assistance and other community-based services.
  • Support clients in navigating systems of care by providing transportation, accompaniment, and advocacy as needed (agency vehicle provided).
  • Maintain accurate, timely documentation in the Homeless Management Information System (HMIS), including case notes, service transactions, and required program forms within 24 hours.
  • Participate in multidisciplinary team meetings and case conferences; provide updates on client progress and coordinate care planning.
  • Build rapport with clients and, when appropriate, support family engagement and reunification efforts.
  • Facilitate life skills development, including independent living, conflict resolution, and self-sufficiency.
  • Attend all required meetings, trainings, and program-related activities.
  • Prepare and submit reports as required by program leadership.
  • Perform other duties as assigned.
  • Support clients in achieving permanent housing outcomes through individualized housing plans and timelines.
  • Conduct Housing Stability Assessments and develop Housing Plans aligned with client goals.
  • Coordinate housing search activities, including unit identification, application support, and placement.
  • Build and maintain relationships with landlords and property management partners.
  • Facilitate housing-focused workshops (e.g., Ready-to-Rent, tenant education, budgeting).
  • Collaborate closely with Income Specialists and Case Managers to align financial and housing strategies.
  • Track housing progress and maintain accurate documentation, including referral logs and housing plans.
  • Participate in intake rotation and multidisciplinary case coordination meetings.
  • Assist clients in obtaining or increasing income through public benefits and employment pathways.
  • Develop and implement individualized Income Plans.
  • Support applications for benefits such as SSI, SSDI, SSA, and General Assistance.
  • Connect clients to employment, education, and workforce development resources.
  • Provide financial literacy education, including budgeting and money management (individual and group formats).
  • Collaborate with Housing Specialists and Case Managers to support overall housing stability.
  • Maintain accurate documentation, including income plans and referral tracking logs.
  • Participate in intake rotation and multidisciplinary case coordination meetings.
