About The Position
Well Care Services is a Behavioral Health Clinic looking for Case Managers to join our team. We are hiring a team of case managers that will be working hands on with patients out on the field. Well Care Services is seeking a full time qualified Mental Health Case Manager to provide services to clients with mental illness.
Requirements
- Minimum of two years of experience working with adults with serious mental illness; or an equivalent combination of training and experience
- Knowledge of serious mental illness, substance abuse, psychotropic drugs, entitlement programs and recovery oriented services required
- Interviewing, service planning, consultation, public relations, and experience with electronic medical record
- Computer skills and the ability to type required
- Good driving record: Must not reflect a total accumulation of six or more negative points within the past three years or a major violation within the past four years
- Must be able to pass a drug and background check
Responsibilities
- Assessing clients' strengths and needs
- Developing individualized service plans
- Completing services assessments
- Linking consumers with other community resources
- Coordinating services including health care
- Monitoring sufficiency of services and goal attainment
- Collaboration with other professionals
- Perform other work as required
