About The Position

Southwest Behavioral and Health Services is seeking a dedicated and empathetic Case Manager for its Flagstaff team. The role provides central support within an interdisciplinary team, primarily serving adults in the Serious Mental Illness (SMI) population. The Case Manager conducts assessments, helps members access community resources, and coordinates care within and outside the agency. The position pays $20.00 per hour. The employer is a nonprofit, community-based behavioral health services provider with more than 115 sites, over 1,000 employees, and 40 years of experience. Requirements

Minimum age of 22.
Bachelor’s degree in a behavioral health field plus one year of relevant behavioral health experience, or a combination of education and experience totaling two years.
Valid Arizona driver’s license and motor vehicle clearance.
Fingerprint clearance issued by the Arizona Department of Public Safety, or eligibility to receive clearance upon hire.
Drug screening is required.

Responsibilities

Complete intake screenings, including intake paperwork and referrals.
Complete annual behavioral health assessments, including service planning and support planning.
Facilitate CFTs and ARTs.
Facilitate groups.
Identify gaps in care for members.
Complete SMI and SED evaluations.
Coordinate care within and outside the agency and assist members in accessing community resources.
