About The Position

Southwest Behavioral and Health Services is seeking a dedicated and empathetic Case Manager to join its Flagstaff team. The Case Manager serves as a central support within an interdisciplinary team, working primarily with adults in the SMI population. The role involves conducting assessments, helping members access community resources, and coordinating care within and outside the agency. The position pays $20.00 per hour. Applicants must be at least 22 years old and meet the education and experience requirements. A valid Arizona driver’s license and motor vehicle clearance are required, as is fingerprint clearance issued by the Arizona Department of Public Safety or eligibility to receive it upon hire. Requirements

Minimum age of 22.
Bachelor’s degree in a behavioral health field plus one year of relevant behavioral health experience, or a combination of education and experience totaling two years.
Valid Arizona driver’s license and motor vehicle clearance.
Fingerprint clearance issued by the Arizona Department of Public Safety, or eligibility to receive clearance upon hire.
Drug screening is required.

Responsibilities

Complete intake screenings, intake paperwork, and referrals.
Complete annual behavioral health assessments, including service planning and support planning.
Facilitate CFTs and ARTs.
Facilitate groups.
Identify gaps in care for members.
Complete SMI and SED evaluations.
Assist members with accessing community resources and coordinate care inside and outside the agency.
