About The Position

The Case Manager manages client, patient, or resident cases using clinical knowledge and expertise and may provide therapeutic services. This is a full-time position with benefits, Monday through Friday. The role provides case management and care coordination within the facility, coordinates treatment and discharge activities, and develops and evaluates care plans. The Case Manager also gathers and communicates clinical information to the treatment team and, as needed, insurance providers and the business office.

Requirements

• Bachelor’s degree in Social Work, Counseling, or a related human service field.
• Current license required within the state where the facility operates, if required.
• CPR and de-escalation/restraint certification required; training is available upon hire and offered by the facility.
• First aid may be required based on state or facility.

Nice To Haves

• One or more years of experience working in a behavioral health setting.
• Previous experience with the patient population served by the facility.
• Previous experience in case management and/or utilization review.

Responsibilities

• Provide case management and care coordination services for clients, patients, or residents in the facility.
• Coordinate continuum-of-care activities for assigned clients and ensure effective resource utilization, service delivery, and compliance with requirements from external agencies and referral sources.
• Conduct psychosocial assessments and individual, family, and group sessions.
• Coordinate treatment and discharge activities as needed.
• Develop, implement, manage, and evaluate client, patient, or resident care plans to provide quality care.
• Collect, correlate, and provide clinical data to the treatment team.
• Communicate medical criteria and clinical information between insurance providers and the treatment team as warranted.
• Correlate clinical data for the business office as required.
• Coordinate issues or activities relevant to the treatment team in a timely manner.
