About The Position

East Side House (ESH) is a community resource in the Bronx and Northern Manhattan that believes education is the key to creating opportunities. ESH’s Adult & Workforce Development Programs provide high-quality programs and services. Reporting to the Employment Coordinator, the Case Manager is a key member of the PSP staff inter-disciplinary team, responsible for providing comprehensive educational and emotional support to enable young adults to meet their personal, educational, and employment goals. The Case Manager will support PSP participants in choosing a post-secondary track and completing outlined track deliverables for certification training, professional soft skills, internship hours, and job placement. This role involves communication with the Assistant Director of PSP and Employment Coordinator to facilitate participant intake, case management, and wrap-around services for various PSP service areas.

Requirements

  • Bachelor’s degree in social work or a related field and experience working with City systems (e.g., justice, public assistance, child welfare, education, housing); OR Associate’s degree and at least three (3) years of successful experience providing case management/counseling and working with City systems.

Responsibilities

  • Manage a caseload of approximately 50–100 participants per year, maintaining regular contact to support progress, engagement, and retention.
  • Conduct participant intake, needs/barrier assessments, and develop individualized service or goal plans, with regular follow-ups and adjustments.
  • Conduct daily/ongoing outreach regarding attendance and engagement, using proactive strategies to reduce barriers and improve persistence.
  • Collaborate with Instructors, Retention Specialist, and Education/Employment Services Managers to deliver coordinated services.
  • Provide one-on-one and small group counseling/coaching to support academic, career, and personal goals, delivering timely feedback.
  • Facilitate workforce readiness and basic life skills workshops aligned to participant needs and program objectives.
  • Complete necessary training for instructor credentials in CPR-First Aid and other competencies.
  • Connect participants to services across the local workforce development system and community resources addressing housing, food security, legal aid, and benefits.
  • Support recruitment and interviewing for new cohorts.
  • Maintain accurate participant files and records in accordance with funder and organizational standards.
  • Build and maintain a shared database for caseload tracking, entering thorough, timely case notes.
  • Prepare reports and participate in data-driven case conferences.
  • Participate in team/department meetings, training courses, case conferences, workshops, and events.
  • Contribute to the continuous improvement of PSP services.
