About The Position

East Side House (ESH) is a community resource in the Bronx and Northern Manhattan that believes education is key to creating opportunities. The Adult & Workforce Development Programs align with this mission by providing high-quality programs and services. The Case Manager is a key role in the PSP staff inter-disciplinary team, responsible for providing comprehensive educational and emotional support to enable young adults to meet their personal, educational, and employment goals. This role supports participants in choosing a post-secondary track and completing deliverables for certification training, professional soft skills, internship hours, and job placement. The Case Manager communicates effectively with the Assistant Director of PSP and Employment Coordinator to facilitate participant intake, case management, and wrap-around services for various PSP service areas.

Requirements

  • Bachelor’s degree in social work or a related field and experience working with City systems (e.g., justice, public assistance, child welfare, education, housing); or Associate’s degree and at least three (3) years of successful experience providing case management/counseling and working with City systems.

Responsibilities

  • Manage a caseload of approximately 50–100 participants per year, maintaining regular contact to support progress, engagement, and retention.
  • Conduct participant intake, needs/barrier assessments, and develop individualized service or goal plans, adjusting plans based on progress.
  • Conduct daily/ongoing outreach regarding attendance and engagement, using proactive strategies to reduce barriers and improve persistence.
  • Collaborate with Instructors, Retention Specialist, and Education/Employment Services Managers to deliver coordinated services.
  • Provide one-on-one and small group counseling/coaching that supports academic, career, and personal goals.
  • Facilitate workforce readiness and basic life skills workshops aligned to participant needs and program objectives.
  • Connect participants to services across the local workforce development system and to community resources addressing housing, food security, legal aid, and benefits.
  • Support recruitment and interviewing for new cohorts; maintain accurate participant files and records.
  • Build and maintain a shared database for caseload tracking; enter thorough, timely case notes; prepare reports and participate in data-driven case conferences.
  • Participate in team/department meetings, training courses, case conferences, workshops, and on/offsite events; contribute to continuous improvement of PSP services.
