Are you passionate about mental health and looking for a meaningful career where you can make a real impact? Legacy Behavioral Health Services is seeking a case manager to join our team in Lowndes County. Under general supervision performs a wide range of tasks in the community to assist clients in regaining control over their own lives while promoting the recovery process. Utilizing psychiatric rehabilitation interventions provides skills training, education and case management to clients and their families. Monitor client progress towards treatment goals. Extensive documentation required; accurately charts services and client progress. Must be able to work well with community agencies and function as part of a team..

Requirements

• Completion of a bachelor’s degree in social work, psychology, mental health, nursing, education, sociology, criminal justice, counseling, rehabilitation counseling, guidance counseling or an equivalent field.
• One year of full-time experience in social services delivery.
• Knowledge of mental illness.
• Valid GA driver’s license with less than 5 points.

Responsibilities

• Monitor client progress towards treatment goals.
• Accurately chart services and client progress.
