About The Position
As a Case Manager-LPN, you facilitate resident admissions and discharges and ensure a seamless transition throughout residents’ time with the organization. You work with residents, their families, and internal and external partners to help ensure residents receive high-quality care and services. The position is identified as Case Manager / Admissions and may be filled by an LPN or RN.
Requirements
- Current state licensure as an LPN, in good standing.
- Ability to read, write, and communicate in English.
Nice To Haves
- Previous admissions or discharge experience is preferred.
Responsibilities
- Conduct pre-admission assessments of prospective residents.
- Maintain continuity of communication with hospitals and community and outlying-service social workers regarding availability within the community.
- Coordinate discharge planning with the social services department.
