As a Case Manager, you will work independently or as part of a team to coordinate a support network and link clients living with mental illnesses, addictions, and physical health issues. The position aims to empower clients, thereby increasing their independence, dignity, and self-respect in a safe and secure environment. Direct service and support will be provided to clients in areas such as daily living, practical assistance, crisis intervention, linkages to primary care, and a range of other services to support individual needs. TAY (Transitional Aged Youth) Co-ops and Supportive Housing provides support to youth facing serious mental health challenges, who might also experience substance use challenges and/or homelessness. This program offers a range of supportive housing options across the city of Toronto Region, either within LOFT-owned houses or through head lease arrangements with private landlords. Each house is shared, fully furnished, and includes support services. TAY Co-ops and Supportive Housing follows a client-centered approach and the psychosocial rehabilitation recovery model – a treatment approach designed to teach emotional, cognitive, and social skills that help those diagnosed with a mental illness live and work in their communities as independently as possible. Community Support Workers and clients meet frequently to design and revise an individualized service plan based on the client’s goals and aspirations. Mental health counseling and harm reduction strategies are often discussed and provided.
