Case Manager I/II
About The Position
This position provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives. The role involves obtaining in-depth social histories, developing and assuring community and/or agency resources to meet the consumer's needs, serving as a Qualified Intellectual Disabilities Professional (QIDP), and providing monthly monitoring of the program. The position requires compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines. Travel is extensive, and the employee must possess a valid driver's license and maintain required car insurance.
Requirements
- All New Employees will complete the Case Manager Academy Training Program.
- Case Manager - Level I: Bachelor's degree in a human services field and one (1) year of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field; OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and one year of experience working directly with individuals with intellectual and/or developmental disabilities. Combination of education and experience.
- Case Manager - Level II: Bachelor's degree in a human services field and two (2) years of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field; OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and two years of experience working directly with individuals with intellectual and/or developmental disabilities. Combination of education and experience.
- For purposes of the Case Manager job family “a bachelor’s degree in a human services field includes any degree from an accredited college or university except for a degree in a physical, natural or biological science or mathematics.
- Must possess a valid driver's license.
- Must maintain required car insurance.
