This position provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives. The role involves obtaining in-depth social histories, developing and assuring community and/or agency resources to meet the consumer's needs, serving as a Qualified Intellectual Disabilities Professional (QIDP), and providing monthly monitoring of the program. The position requires compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines. Travel is extensive, and the employee must possess a valid driver's license and maintain required car insurance.
