About The Position

Helio Health is seeking a Case Manager to support its mission of promoting recovery from the effects of substance use and mental health disorders and other health issues. The role manages and coordinates services for patients and residents participating in or having completed treatment programs. This is a full-time position in the HUD Housing program, with a Monday–Friday schedule from 8:00 a.m. to 4:30 p.m. Home visits are required; agency vehicles may be provided. Compensation is $22.00–$24.00 per hour. The position is eligible for a sign-on incentive of up to $1,500.

Requirements

• High school diploma or GED.
• Valid New York State driver’s license.

Nice To Haves

• Associate degree preferred.
• Two years of experience working with behavioral health and/or substance abuse preferred.

Responsibilities

• Maintain a caseload and provide primary case work, including screening, intake, orientation, assessment, intervention, treatment planning, referrals, crisis intervention, patient/resident education, case management, reporting, record keeping, consultation, care coordination, and discharge planning.
• Train staff, volunteers, and students/interns.
• Conduct required home visits; agency vehicles may be provided.
• Perform quality assurance activities.
• Provide services in accordance with agency policies, procedures, program guidelines, and governmental regulations.
• Monitor the provision of services when assigned.
• Evaluate programs and identify strengths and weaknesses.
• Assist with problem resolution and implementation of supervisor-approved program modifications.
• Participate in program administrative and clinical committees as assigned.
• Represent the agency in the community.
• Perform other duties as assigned by the supervisor.
