About The Position

Bridges Healthcare, Inc. is hiring a Case Manager in its Community Support Program. The role supports activities and programming within the Community Support Services division to help individuals become empowered in their recovery and increase independence. The position is full-time, with a 35-hour work week, and pays $22.46–$28.87 per hour, commensurate with experience.

Requirements

Bachelor’s degree required; a major in human services is preferred.
Ability to work a flexible schedule.
Knowledge of behavioral health issues and understanding of community integration strategies and techniques.
Good communication and documentation skills.
Computer literacy.
Ability to prioritize and organize schedules and demands.
CPR/First Aid certified or willing to become certified.
Valid driver’s license and access to reliable transportation.

Responsibilities

Facilitate recovery-focused activities to motivate consumers toward improving their quality of life.
Assist consumers in developing crisis plans and advance directives for emergency or crisis situations.
Provide advocacy and education concerning treatment barriers and stigma.
Network and collaborate with community resources and providers.
Assist in developing consumers’ treatment plans.
Maintain client records.
Assist in developing program and client budgets.
Supervise, coach, and provide guidance to supervisees.
Perform other department- or agency-related duties as directed by the supervisor.
