About The Position
Alternatives, Inc. is hiring a full-time Case Manager for its Community Support Services (CSS) program, a clinical case management program serving people diagnosed with mental illness who live independently in the community, in Agency-owned housing or their own residences. Case Managers support individuals in working toward personal goals, meeting their needs, and enhancing their quality of life. The position is based in Raritan, New Jersey, and includes in-home and on-site services. Employees complete paid onboarding and ongoing training, some of which takes place at the main office in Raritan and some remotely.
Requirements
- Bachelor's degree in a human-services-related field.
- Experience working with individuals who have mental illness.
- Computer skills.
- Valid driver's license from New Jersey, New York, or Pennsylvania.
- Driving record with no more than five points and no DUI within the last three years.
- Pass criminal and registry background checks.
- Successfully complete and pass a drug screen.
- Clear tuberculosis screening.
- Complete 10–15 trainings upon hire and on an ongoing basis.

- Experience with an electronic health record system and Medicaid reimbursement programs.
Responsibilities
- Provide in-home and on-site services and support according to each individual's treatment plan.
- Provide direct client services in areas including motivational interviewing, money management, daily living skills, symptom improvement, psycho-education, housing support, and job skills training.
- Evaluate each individual's skills and needs and make recommendations to the treatment team.
- Learn the eight dimensions of wellness and multiple interventions to provide quality Medicaid services.
- Use an electronic health record (EHR) system to write clear, concise progress notes for Medicaid-reimbursable services.
- Provide crisis intervention as necessary and develop crisis prevention plans to improve crisis response.
- Complete all Agency- and program-specific trainings.
