About The Position

Case Managers at NewYork-Presbyterian/Queens are responsible for leading the full spectrum of patient care from pre-admission to discharge, ensuring quality outcomes. This role involves intensive care management for adult patients with complex medical needs, serving as a liaison between the interdisciplinary team (social workers, physicians, therapists, etc.) to direct inpatient care, develop individualized care plans, and optimize length of stay. The position aims to prevent hospital readmissions and anticipate outpatient service needs by setting appointments and consultations prior to discharge. Case Managers foster a collaborative relationship with medical staff to achieve case management objectives, reduce length of stay, and provide quality patient care across the continuum.

Requirements

Bachelors degree in Nursing (BSN)
Current NYS Registered Nurse (RN) licensure
3 years clinical experience as a registered nurse in a medical or surgical unit, Home Care or post-acute facility

Nice To Haves

2 years of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting
1 year of leadership experience in a healthcare setting
Master's Degree in healthcare related field
Case Management Certification

Responsibilities

Providing intensive care management for adult patients with complex medical needs.
Serving as a liaison between the interdisciplinary team of social workers, Physicians, Physical Therapists, Occupational Therapists, Cardiologists, Intensivists.
Directing all aspects of inpatient care.
Developing individualized care plans.
Ensuring optimal length of stay.
Helping prevent hospital readmission.
Anticipating outpatient service needs, setting appointments and consultations prior to discharge.
Promoting a collaborative relationship with the medical staff to accomplish case management objectives to reduce length of stay (LOS) and provide quality patient care across the continuum of care.
