About The Position

Case Managers at NewYork-Presbyterian/Queens are the guardians of continuity of care, leading the full spectrum of patient care from pre-admission to discharge while ensuring quality outcomes. As a Care Manager, you will serve as a highly valued and respected leader, empowered to make amazing things happen for patients and their families. You will be responsible for providing intensive care management for adult patients with complex medical needs and serve as a liaison between the interdisciplinary team (social workers, Physicians, Physical Therapists, Occupational Therapists, Cardiologists, Intensivists) to support patients and their families. You will direct all aspects of inpatient care, develop individualized care plans, ensure optimal length of stay, help prevent hospital readmissions, and anticipate outpatient service needs by setting appointments and consultations prior to discharge. Case Manager activity promotes a collaborative relationship with the medical staff to accomplish case management objectives, reduce length of stay (LOS), and provide quality patient care across the continuum of care.

Requirements

  • Bachelors degree in Nursing (BSN)
  • Current NYS Registered Nurse (RN) licensure
  • 3 years clinical experience as a registered nurse in a medical or surgical unit, Home Care or post-acute facility

Nice To Haves

  • 2 years of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting
  • 1 year of leadership experience in a healthcare setting
  • Master's Degree in healthcare related field
  • Case Management Certification

Responsibilities
