About The Position

Case Managers at NewYork-Presbyterian/Queens are skilled clinicians leading the full spectrum of patient care, from pre-admission to discharge, ensuring quality outcomes. As a Care Manager, you'll serve as a valued leader, empowered to make a difference for patients and their families. This role fosters a culture of autonomy, pride, compassion, and expertise. You will be responsible for providing intensive care management for adult patients with complex medical needs, serving as a liaison between the interdisciplinary team (social workers, Physicians, Physical Therapists, Occupational Therapists, Cardiologists, Intensivists) to support patients and their families. You will direct all aspects of inpatient care, develop individualized care plans, ensure optimal length of stay, help prevent hospital readmissions, and anticipate outpatient service needs by setting appointments and consultations prior to discharge. Case Managers promote a collaborative relationship with medical staff to achieve case management objectives, reduce length of stay (LOS), and provide quality patient care across the continuum of care.

Requirements

  • Bachelors degree in Nursing (BSN)
  • Current NYS Registered Nurse (RN) licensure
  • 3 years clinical experience as a registered nurse in a medical or surgical unit, Home Care or post-acute facility

Nice To Haves

  • 2 years of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting
  • 1 year of leadership experience in a healthcare setting
  • Master's Degree in healthcare related field
  • Case Management Certification

Responsibilities
