Requirements
- Licensed LVN
- Previous Case Management experience
- Computer skills
- Communication skills
- Problem-solving skills and attention to detail
- Ability to multi-task, work independently
- Able to follow and manage multiple patient cases concurrently
Responsibilities
- Promoting the efficient utilization of skilled services by ensuring that patients are admitted according to medical necessity.
- Ensuring that services are provided to meet the assessed need of each patient in a prompt and efficient manner, and in accordance with patients insurance.
- Facilitating that patients are discharged to the most appropriate environment with the necessary supportive services to meet their ongoing care needs.
- Ensuring facility receives proper authorization of care and level of service.
