About The Position

The Case Manager I at Behavioral Health Services Inc is responsible for providing comprehensive case management services to clients with behavioral health needs. Requirements

  • Experience working with diverse populations and understanding of cultural competence in service delivery.
  • Prior experience in developing and implementing individualized care plans.
  • Strong organizational skills with the ability to manage multiple cases efficiently.
  • Excellent communication skills, both verbal and written, to effectively interact with clients, families, and healthcare professionals.
  • Ability to conduct thorough assessments and develop comprehensive care plans.
  • Proficiency in using case management software and maintaining accurate client records.
  • Strong problem-solving skills and the ability to work independently as well as part of a multidisciplinary team.
  • Demonstrated ability to maintain confidentiality and adhere to ethical standards in all interactions.
  • Familiarity with community resources and ability to facilitate referrals and coordinate services for clients.

Responsibilities

  • Conduct thorough assessments of clients' needs, strengths, and challenges to develop individualized care plans.
  • Coordinate and monitor the delivery of services, ensuring that clients receive appropriate interventions and support.
  • Advocate for clients' needs and rights within the healthcare system and community resources.
  • Facilitate access to medical, psychological, and social services by collaborating with healthcare providers, community organizations, and other stakeholders.
  • Maintain accurate and up-to-date documentation of client interactions, progress, and outcomes in compliance with organizational and regulatory standards.
  • Provide crisis intervention and support to clients as needed, ensuring their safety and stability.
  • Educate clients and their families about available resources and support systems to empower them in their recovery journey.
  • Participate in multidisciplinary team meetings to review client progress and adjust care plans as necessary.
  • Stay informed about current trends and developments in behavioral health to enhance service delivery and client outcomes.
