The Case Manager 2 provides home- and community-based case management services to homeless young adults, ages 18–24, throughout Connecticut through the START program. The program combines short-term rental assistance with intensive case management to support housing stabilization, using trauma-informed care and positive youth development. The Case Manager works with participants to identify goals and resources, exit homelessness, and achieve long-term stability. The caseload varies between 8 and 12 young adults. The role also works with Young Adult Services and START staff to help stabilize and reunite families experiencing complex issues, including substance use, behavioral health matters, and homelessness. Newly hired employees must attend a mandatory two-day orientation at The Connection’s Roscommon office in Middletown.
