About The Position
The Case Facilitator I will be a member of the DeKalb Outpatient treatment team. The role provides case management and activities-of-daily-living skills to adult consumers, conducts client reassessments and completes ANSA, recovery plans and reviews, and provides referral and linkage to services and client release.
Requirements
- High school diploma or equivalent.
- Must meet OBHP requirements.
- Must possess a valid state-issued driver's license.
Nice To Haves
- Two years’ experience in mental health or community services is preferred.
Responsibilities
- Provide case management to adult consumers.
- Provide activities-of-daily-living skills to adult consumers.
- Conduct client reassessments and complete ANSA.
- Complete recovery plans and reviews.
- Provide referrals and linkage to services.
- Complete client release.
