About The Position
The Case Facilitator II is a member of a multidisciplinary treatment team. The role includes reassessing clients and completing the CANS instrument, assigning cases appropriately for direct service, monitoring client treatment episodes, recovery plans and reviews, referring and linking clients to services, and managing client terminations.
Requirements
- Bachelor’s degree related to mental health.
- Must meet OBHP requirements.
- Must possess a valid state-issued driver’s license.
Nice To Haves
- Two years’ experience in mental health or community services preferred.
