About The Position

Community Action Coalition for South Central Wisconsin works to develop the economic and social capacities of individuals, families, and communities in Dane, Jefferson, and Waukesha Counties. The Case Coordinator develops relationships with participants, assesses their needs, and connects them to wraparound services. The position also develops intake and assessment processes and coordinates CAC programs and services across three counties using motivational interviewing and other communication techniques. Coordinators build community connections so people in need can access CAC programs and services, track and lead program outcomes across their service area, and handle reporting and outcomes associated with program oversight. The ideal candidate respects individual dignity and is committed to social justice and equity.

Responsibilities

• Develop relationships with participants to assess their needs and connect them to wraparound services.
• Develop processes for intake and assessment.
• Use motivational interviewing and other communication techniques to carry out client intake, assessment, and coordination of CAC programs and services across three counties.
• Develop connections across communities so community members in need can access CAC programs and services.
• Track and take the lead for program outcomes across the service area.
• Manage reporting and outcomes associated with oversight of assigned programs.
