About The Position

The Case Coordinator develops relationships with participants to assess their needs and connect them with wraparound services. The role develops intake and assessment processes and uses motivational interviewing and other communication techniques to coordinate client intake, assessment, and Community Action Coalition (CAC) programs and services across three counties. The Case Coordinator builds community connections so that people in need can access CAC programs and services, and leads tracking and reporting of program outcomes across the service area. The ideal candidate respects individual dignity and is committed to social justice and equity.

Responsibilities

• Build relationships with participants to assess their needs and connect them with wraparound services.
• Develop processes for intake and assessment.
• Use motivational interviewing and other communication techniques to implement client intake, assessment, and coordination of CAC programs and services across three counties.
• Develop community connections to help community members in need access CAC programs and services.
• Track and lead program outcomes across the service area.
• Oversee program reporting and outcomes.
