Caregiver - CNA/HHA/PCA (Manchester)
About The Position
Hartford HealthCare Independence at Home provides care that helps elderly and disabled individuals achieve maximum independence, participate in their plans of care, and live with dignity in their own homes. Its highly trained staff take a holistic approach to care, considering clients’ family and living situations when planning appropriate support. The Homemaker Caregiver provides personal care, assistance with daily activities, meal preparation, routine housekeeping and home management, and emotional support for clients and their families. The caregiver records care in each client’s care plan, reports changes in condition to the appropriate care team member, and may assist with routine therapy exercises and participate in case conferences. The role requires professional customer service, critical thinking without direct supervision, and escalation of issues when appropriate.
Requirements
- A driver’s license, a car, and auto insurance.
- One year of previous personal care experience at a facility or agency.
- Two years of continuous work experience.
- Comfort with driving within 30 minutes of home for cases.

Responsibilities
- Provide personal hygiene and grooming care to clients.
- Assist clients with toileting, ambulation, and other needs.
- Provide meals that meet clients’ nutritional needs.
- Report changes in clients’ conditions to a nurse, supervisor, or client care coordinator.
- Perform routine housekeeping and home management duties.
- Record each client’s care on the care plan and submit the care plan in a timely manner.
- Assist with providing emotional support to clients and their families.
- Assist with routine therapy exercises.
- Participate in case conferences as requested.
- Attend all required in-service education sessions.
- Use computer software and a smartphone for required education.
- Represent the company positively and professionally through a high level of customer service.
- Think critically without direct supervision and escalate issues when appropriate.
