PT Evening LPN/RN - Care Nurse Supervisor

About The Position

Benchmark is seeking a compassionate Licensed Practical Nurse (LPN) to provide nursing care to residents in a warm, comfortable, home-like environment. The role is part of a supportive team focused on building community and lasting relationships with residents and associates. The position is scheduled from 3 p.m. to 11 p.m., including every other weekend on both Saturday and Sunday. Rates start at $34.00 per hour. Benefit eligibility may vary by employment status.

Requirements

Current state LPN license and CPR certification.
Graduate of an approved LPN program, as required by the state.
Minimum of 1 to 2 years of experience working as a nurse is preferred.
Previous experience working with elderly people in a paid or volunteer position is highly desirable.

Responsibilities

Accurately assess, communicate, and document residents’ status.
Observe and report significant changes in resident behavior and health to the Resident Care Director, physician, family, RCAs, and Executive Director, in accordance with state regulations.
Direct and supervise the daily work assignments of Resident Care Assistants.
Mentor, guide, and direct Lead Resident Care Assistants in all aspects of their jobs.
Coordinate care needs with community providers through an effective case management process.
Balance service demands with support for resident independence.
Recognize and help prevent elder abuse, neglect, and exploitation, including financial exploitation, and report concerns to appropriate sources in accordance with state regulations.
Respond to emergencies and personal emergency response system alerts, and know when to call for backup.
