About The Position

The Care Manager assists residents with all aspects of daily living identified in their care plans, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities. This is a regular part-time, hourly, non-exempt position scheduled for 22.5 to fewer than 30 hours per week, on the second shift from 2:30 p.m. to 10:30 p.m. The compensation is up to $22 per hour.

The role reports to the Health & Wellness Director. The position requires physical activity, including standing, walking, sitting, reaching, balancing, stooping, kneeling, crouching, and lifting or supporting residents. Employees must use proper body mechanics.

Requirements

• Certified Nursing Assistant or Certified Home Health Aid.
• Knowledge and experience working with the elderly population.
• Ability to lift or carry up to 50 pounds and to lift and support residents, using proper body mechanics.

• Dementia experience.

Responsibilities

• Provide resident care, including bathing, dressing, toileting, and feeding, as outlined in each resident’s care plan.
• Maintain residents’ safety, comfort, dignity, and confidentiality.
• Escort residents to meals and activities.
• Ensure assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts.
• Document all services provided to assigned residents during the shift.
• Immediately report any change in a resident’s condition to a supervisor.
