About The Position

This position involves supervising and supporting Care Managers to ensure timely documentation, regulatory compliance, and high-quality client care. The role requires managing caseloads, overseeing electronic health records, providing coaching and performance feedback, and collaborating with internal teams and community partners. The supervisor will also be responsible for scheduling and tracking training, facilitating staff development, and managing staff schedules. This is an office/field-based position covering Nassau & Suffolk County and the 5 boroughs of NYC, with in-person or virtual meetings held at either the Brooklyn or Dix Hills location. New York residency is required.

Requirements

  • Bachelor’s degree required with minimum 4+ years of experience in child welfare, care management, or a related field
  • 2–5 years of supervisory or managerial experience
  • Experience in Health Home, care coordination, or case management programs
  • Strong knowledge of DOH regulations, Health Home standards, and compliance requirements
  • Complete Children Adolescent Needs Assessment (CANS)NY training
  • Be CANS-NY certified annually
  • Meet exam score requirements: Supervisors: minimum 80%
  • Strong leadership, staff development, and performance management skills
  • Excellent communication, organization, and problem-solving abilities
  • Experience with electronic health records (e.g., Foothold) and documentation oversight
  • Ability to manage multiple priorities, deadlines, and audits
  • Strong crisis intervention and decision-making skills
  • New York residency required

Nice To Haves

  • Master’s degree preferred in Social Work, Nursing, Public Health, or related field
  • Care Managers: minimum 70% exam score

Responsibilities

  • Supervise and support Care Managers through regular individual and team supervision
  • Oversee caseloads to ensure timely documentation and compliance with regulatory requirements
  • Review and maintain electronic health records (EHR) for accuracy and completeness
  • Provide coaching, performance feedback, and professional development support
  • Assist with audit preparation and ensure program readiness
  • Participate in field visits, case conferences, and interdisciplinary team meetings (IDT)
  • Manage staff schedules and coverage; provide direct services as needed
  • Collaborate with internal teams and community partners to support client outcomes
  • Schedule and track required trainings (DOH, Compliance, HIPAA, Health Homes, Relias, others)
  • Ensure completion of mandated annual trainings
  • Facilitate staff training and provide program updates
  • Monitor and communicate policy or program changes to staff and leadership
  • Provide real-time coaching, field-based supervision, and case guidance
  • Deliver performance evaluations, corrective action, and recognition (with HR support)
  • Promote leadership development and problem-solving skills among staff
