About The Position

This part-time position is responsible for the provision of care management services for older adults residing in the community. Tasks include providing care management assessments and developing strengths-based care plans, as well as linking older adults and their families with community resources. You will also collaborate and build relationships with community providers.

Requirements

  • Bachelor’s degree in social work, social services, human services, health or nursing or other closely related field
  • Ability to pass a background check, including the Adult Abuse Registry and clean driving record
  • Valid driver’s license, proof of auto insurance, and a clean driving record
  • Persons with mental or physical disabilities are eligible if they can perform essential functions of the job after reasonable accommodation is made to their known limitations. Responsibilities

  • Conducts in-home evaluations to assess all aspects of the older adult’s life, e.g., general health, medical providers, medications, cognitive ability, nutrition, home safety, social supports, etc.
  • Develops and implements personalized care plans designed to assure client safety and independence as well as enhance overall well-being
  • Responds to client crises as needed
  • Provides resources and referrals for other support services (e.g., in-home aides, housekeeping, financial advisor, estate planning, etc.) and recommends changes as appropriate.
  • Conducts wellness monitoring through home visits and frequent phone contact to evaluate well-being and unmet safety concerns.
  • Accompanies client to doctor appointments to ensure that concerns are expressed, and medical issues are understood
  • Provides caregiver and family consultation, including helping them navigate available resources as well as state and federal benefits, identifying appropriate options, and selecting the course of action to ensure their loved one’s well-being and their own peace of mind
  • Assists in all service coordination activities outlined in the care plan
  • Conducts community outreach designed to engage potential referral sources
