Care Coordinator - Davita IKC - Bainbridge, GA
About The Position
DaVita Integrated Kidney Care is hiring a Care Coordinator to help patients with complex chronic conditions navigate care and access resources. The position works closely with Nurse Practitioners, RN Case Managers, and other care-team members to coordinate services, support referrals and appointments, and help patients move through the continuum of care. The role is full-time, Monday through Friday. It requires commuting to DaVita clinics in the designated region three to four times per week, with the remaining work time from home. The daily start time is between 7:00 and 8:00 a.m., with flexibility as needed to meet patient and clinic needs. Reliable transportation is required, and mileage is reimbursed. The remote workspace must be quiet and private, with high-speed internet.
Requirements
- High school diploma or GED.
- At least 2 years of experience in a professional office environment.
- Ability to handle confidential information with discretion and sound judgment.
- Strong verbal communication skills.
- Proficiency in Microsoft Office, Excel, and Outlook.
- Commitment to service excellence and patient satisfaction.
- Ability to work autonomously while collaborating effectively with the team.
- A quiet, private home workspace with reliable high-speed internet.
- Reliable transportation for travel to DaVita clinics in the designated region.
Nice To Haves
- Prior experience with Electronic Medical Records.
- 1–3 years of clinical medical office experience.
- CNA or MA experience.
- Experience coordinating care, including referrals or care management.
- Familiarity navigating insurance payers.
- Experience in direct patient care or support.
- Prior experience in a medical office environment.
Responsibilities
- Coordinate patient care by helping patients access eligible resources and programs and assisting with referrals and provider appointments.
- Make outbound calls and respond to inbound calls from patients, providers, and Village resources.
- Complete non-clinical screenings and pathways under the Manager’s guidance, escalating to the Integrated Care Nurse when needed.
- Assist the clinical team in moving patients through the continuum of care by completing assigned tasks.
- Identify eligible resources and collaborate with Integrated Care Nurses, dialysis clinics, health plans, and other stakeholders to help members enroll in programs.
- Communicate with patients in a timely and professional manner.
- Maintain accurate, complete, and confidential documentation.
- Demonstrate effective two-way oral and written communication.
