About The Position

Mae Health is building a pipeline of Care Coordinators to support members across its Medicaid health plan partnerships. The role helps mothers, babies, and families navigate healthcare services, address barriers to care, connect with clinical and community resources, and receive support throughout their maternal health journey. This is a future-opportunity posting and does not represent a currently approved open position; assignment duration and any potential extension will be confirmed if an approved position becomes available, and extension or transition to regular employment is not guaranteed. Care Coordinators conduct comprehensive needs assessments; coordinate care plans, referrals, and follow-up with payer care management teams; maintain accurate documentation; and liaise among members, Mae teams, and external partners. They build trusted relationships through compassionate, culturally responsive communication, encourage member participation, and escalate high-risk factors and care concerns. The role requires balancing member support with consistent outreach, timely coordination, accurate records, and operational goals. The position is a full-time, 40-hour-per-week W-2 temporary role, fully remote within Mae-approved U.S. hiring states. The schedule is Monday through Friday, with flexible scheduling potentially available and occasional evening hours possibly required. Compensation is $32.00–$38.00 per hour, depending on experience and qualifications. The employee must have reliable internet access and a private workspace suitable for confidential member conversations.
