About The Position

Join the Gastroenterology team at 87th Ave as a full-time Care Coordinator! The Care Coordinator provides complex and diversified support to the Care Team (provider, medical assistant, nurse, etc.) by coordinating care for patients on the daily schedule and will assist with rooming two days per week. Working from a team approach, and in continuous partnership with patients and providers to promote timely access to needed care, comprehension and continuity of care, and the enhancement of patient well-being.

Requirements

• Graduate of an accredited Medical Assistant program.
• Certification from national certifying entity (AAMA, NCCT, AMT, NHA, etc.)
• Current Washington State Medical Assistant-Certified credential required, or ability to obtain within 6 months of hire.
• Current healthcare provider BLS required.
• At least two years relevant experience in a clinical setting required, outpatient care and service of patients with chronic disease preferred.
• Ability to work, with or without reasonable accommodation, with a diverse population of patients and colleagues seeking or considering care in all areas.

Responsibilities

• Serve as an effective communication link between patient and clinic staff by gathering information from patients.
• Independently and proactively identify patients by running panel reports for specific diagnosis and preventive care needs.
• Evaluate and analyze patient records for accurate pre-visit data.
• Communicate with patients directly to discuss preventative care needs and refers patients with medical concerns to appropriate clinical staff.
• Review provider schedules and patient charts to assist the care team in coordinating care for visits and identifying preventive healthcare needs.
• Work with clinic team on developing standards, implementation, and maintenance of programs in chronic disease management for patients.
• Assist in rooming patients as needed.
