About the position
The job overview for the role of Care Coordinator at Dialogue is to act as a primary point of contact for members, supporting operations throughout the day. The Care Coordinator will welcome new patients, verify and follow up on active cases, assist practitioners with administrative tasks, and help patients navigate the platform and healthcare system. They will also be responsible for critically reviewing the program and generating ideas for process optimization and improving the patient experience. Strong communication skills, empathy, attention to detail, and a passion for health and quality of care are essential for this role. Fluency in French, knowledge of the public health system, and availability to work evenings and alternate weekends are preferred qualifications.
Responsibilities
- Welcoming new patients to the program and accompanying them on their journey
- Verifying and following up on active cases and assisting practitioners in their administrative tasks
- Finding the best resource for the patient to deliver a memorable experience
- Helping the patient navigate our platform and the public and private health care system
- Critically reviewing the program and generating ideas to optimize processes and patient experience
- Ensuring total customer satisfaction
- Passion for health, quality of care, and improving team processes and understanding
- Strong sense of empathy, time management, critical thinking, attention to detail, and excellent customer service
- Ability to communicate in French, both written and spoken
- Education or work experience in a health-related field is an asset
- Availability to work evenings and alternate weekends
- Strong computer skills
- Knowledge of the public health system (an asset)
