About the position
The job overview for the role of Care Coordinator at Dialogue is to act as a primary point of contact for members, supporting operations throughout the day. The coordinator will welcome new patients, verify and follow up on active cases, assist practitioners with administrative tasks, and help patients navigate the platform and healthcare system. They will also review the program and generate ideas for optimization, while ensuring total customer satisfaction. Strong communication skills, empathy, and a passion for health are essential for this role.
Responsibilities
- Welcoming new patients to the program and accompanying them on their journey
- Verifying and following up on active cases and assisting practitioners in their administrative tasks
- Finding the best resource for the patient to deliver a memorable experience
- Helping the patient navigate the platform and the public and private healthcare system
- Critically reviewing the program and generating ideas to optimize processes and patient experience
- Ensuring total customer satisfaction
- Having a passion for health, quality of care, and improving team processes and understanding
- Demonstrating a strong sense of empathy, time management, critical thinking, attention to detail, and excellent customer service
- Communicating in French, both written and spoken
- Having education or work experience in a health-related field (an asset)
- Availability to work evenings and alternate weekends
- Possessing strong computer skills
- Knowledge of the public health system (an asset)
