Business Development Specialist- Home Care
About The Position
PurposeCare is seeking a dependable Marketing Specialist, also described as a Care Transition Coordinator, to support referrals and company growth. The role develops the company’s Home Health and Home Care presence within supportive living and home- and community-based residences. It generates referrals by building relationships with hospitals, healthcare systems, case managers, and other community resources. The position represents the agency professionally and compassionately, supports the complete referral and care coordination process, and upholds Purpose Home Health’s policies and procedures.
Requirements
- At least one year of home health, home care, marketing, or case management experience, or a combination of these.
- Valid driver's license.
- Excellent oral, written, and active listening skills.
- Ability to work independently, maintain confidentiality, and meet assigned deadlines.
- Sound judgment and good decision-making skills.
- Ability to organize and prioritize work.
- Ability to establish good working relationships internally and externally.
- Ability to sit at a desk and listen effectively on the telephone for extended periods.
- Professional appearance and demeanor; patient and friendly manner.
- Proficient computer skills and ability to use office equipment.
- Ability to work evenings or weekends as required.

Responsibilities
- Participate in short- and long-range planning to achieve company goals and objectives.
- Manage the complete referral process, including identifying home health needs, verifying benefits, and coordinating care.
- Cultivate new and existing relationships that contribute to company growth.
- Develop the Home Health and Home Care presence within supportive living and home- and community-based residences.
- Generate new referrals by building relationships with area hospitals, healthcare systems, case managers, and other community resources.
- Represent the agency in a professional, prepared, and caring manner.
- Prepare weekly reports on marketing activities.
- Initiate and expand the referral network.
- Comply with company compliance and HIPAA regulations.
- Perform other duties as assigned.
