About The Position

PurposeCare is seeking a dependable Marketing Specialist to support the referral process for its home care and home health services. PurposeCare provides home-based care to more than 3,000 clients each month, helping people remain healthy and safe at home.

The Care Transition Coordinator is an experienced field marketer responsible for company growth. The role develops PurposeCare’s Home Health/Home Care presence within supportive living and home- and community-based residences, generating referrals by building relationships with area hospitals, healthcare systems, case managers, and other community resources. The position is responsible for coordinating the referral process, identifying home health needs, verifying benefits, and coordinating care.

The professional is expected to uphold PurposeCare policies and procedures, maintain confidentiality, communicate effectively, exercise sound judgment, organize and prioritize work, and establish strong internal and external working relationships. The position may require evening or weekend work.

Requirements

• At least one year of Home Health/Home Care, marketing, or case management experience, or a combination of these.
• Must possess a valid driver's license.
• Demonstrate excellent oral, written, and active listening skills.
• Ability to work independently, maintain confidentiality, and meet assigned deadlines.
• Demonstrate sound judgment and good decision-making skills.
• Uphold integrity and fair-mindedness consistent with company standards, practices, policies, and procedures.
• Ability to organize and prioritize work.
• Ability to establish good working relationships internally and externally.
• Ability to sit at a desk and listen effectively for long periods on the telephone.
• Ability to present a professional appearance and demeanor.
• Ability to use office equipment.
• Must be patient and friendly.
• Proficient computer skills.
• Ability to work evenings or weekends as required.

Responsibilities

• Participate in short- and long-range planning to achieve company goals and objectives.
• Manage the complete referral process, including identifying home health needs, verifying benefits, and coordinating care.
• Cultivate new and existing relationships to support company growth.
• Develop the company's Home Health/Home Care presence within supportive living and home- and community-based residences.
• Generate referrals by building relationships with area hospitals, healthcare systems, case managers, and other community resources.
• Represent the agency in a professional, prepared, and caring manner.
• Prepare weekly reports on marketing activities.
• Initiate and expand the referral network.
• Comply with company compliance requirements and HIPAA regulations.
• Perform other duties as assigned.
