About The Position

Merakey is seeking a Blended Case Manager to join its team around Monaca, Pennsylvania. Under the direction of the Blended Case Management Supervisor, the role serves as the primary contact for up to 30 individuals in the community. The position includes developing, coordinating, and implementing personalized service plans; providing case management and supportive counseling; and coordinating with other agency providers. The Case Manager also works alongside county reentry grants to support individuals transitioning from jail back into the community and participates in on-call responsibilities as needed. The stated pay is $23.19 per hour with selection of the Enhanced Pay Option.

Requirements

• High school diploma or GED and at least 12 semester credit hours in sociology, social welfare, psychology, gerontology, or another social science.
• At least 2 years of experience in public or private human services, including 1 year of direct care experience; experience must be with clients over age 18.
• Valid driver's license.
• All clearances required by OMHSAS regulations.
